Anyone under the age of 18 must be accompanied by a parent or legal guardian at the time of their appointment.

If you are unable to keep your appointment, we kindly ask that you provide 24 hour notice so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery. We will keep you informed of delays and reschedule appointments as necessary.

We utilize NetReminder, an electronic telephone patient appointment reminder system as another added convenience. NetReminer will call your designated number to remind you of your appointment time. If you do not want the automatic reminder, please notify the office upon scheduling.

Prior to Your Appointment

At least one working day prior to your first appointment, please provide our office with the following information:

- Current insurance information and cards
- Any recent medical records and/or x-rays from other physicians pertaining to the problem
- Completed Patient Registration Form (see Patient Forms)
- Completed Health History Form (see Patient Forms)
- Acknowledgement of Receipt of Privacy Notice (see Patient Forms)
- Referral Form from Primary Care Provider (PCP) if necessary
- Referral number if insurance required
- Drivers license or photo ID

OSSL medical forms should be completed and submitted prior to initial appointment. New patients who cannot submit required forms prior to visit must arrive 30 minutes prior to scheduled appointment time in order to complete. We utilize electronic medical records and all completed forms and data must be scanned for physician availability prior to appointment time.
